Responsibilities
- Conduct virtual patient assessments and triage via video, phone, and secure messaging platforms.
- Develop and implement individualized care plans in collaboration with physicians and interdisciplinary team members.
- Educate patients and families on disease management, medication adherence, and preventive health measures.
- Document all encounters accurately in the electronic health record (EHR) in compliance with state and federal regulations.
- Participate in quality‑improvement initiatives and telehealth performance metrics.
Requirements
- Active Ohio Registered Nurse (RN) license.
- Minimum 2 years of clinical nursing experience (acute care, primary care, or case management preferred).
- BSN degree or higher; ADN with relevant experience will be considered.
- Proficiency with telehealth platforms, EHR systems (Epic/Cerner), and basic troubleshooting of audio/video equipment.
- Excellent communication skills, empathy, and ability to work independently.
- Current BLS certification; ACLS preferred.
