Responsibilities

  • Conduct virtual patient assessments and triage via video, phone, and secure messaging platforms.
  • Develop and implement individualized care plans in collaboration with physicians and interdisciplinary team members.
  • Educate patients and families on disease management, medication adherence, and preventive health measures.
  • Document all encounters accurately in the electronic health record (EHR) in compliance with state and federal regulations.
  • Participate in quality‑improvement initiatives and telehealth performance metrics.

Requirements

  • Active Ohio Registered Nurse (RN) license.
  • Minimum 2 years of clinical nursing experience (acute care, primary care, or case management preferred).
  • BSN degree or higher; ADN with relevant experience will be considered.
  • Proficiency with telehealth platforms, EHR systems (Epic/Cerner), and basic troubleshooting of audio/video equipment.
  • Excellent communication skills, empathy, and ability to work independently.
  • Current BLS certification; ACLS preferred.
