Responsibilities

Conduct virtual patient assessments and triage via video, phone, and secure messaging platforms.
Develop and implement individualized care plans in collaboration with physicians and interdisciplinary team members.
Educate patients and families on disease management, medication adherence, and preventive health measures.
Document all encounters accurately in the electronic health record (EHR) in compliance with state and federal regulations.
Participate in quality‑improvement initiatives and telehealth performance metrics.

Requirements

Active Ohio Registered Nurse (RN) license.
Minimum 2 years of clinical nursing experience (acute care, primary care, or case management preferred).
BSN degree or higher; ADN with relevant experience will be considered.
Proficiency with telehealth platforms, EHR systems (Epic/Cerner), and basic troubleshooting of audio/video equipment.
Excellent communication skills, empathy, and ability to work independently.
Current BLS certification; ACLS preferred.
